A. How to request your health records
This section is provided under Texas Health and Safety Code §181.105 (House Bill 4224) and applies to all patients.

- How to submit a request: Send a written request by message in the Practice Better patient portal, by email to [email protected], or by mail to 8088 Old Austin Rd. Unit 2A, Selma, TX 78154.
- What to include: your full name, date of birth, phone or email, the records you want (all records or specific items and dates), the format you want (electronic or paper), and where to send them. We will verify your identity before releasing records. A personal representative must include proof of authority, such as a medical power of attorney or guardianship papers.
- When you will receive them: within 15 business days of our receiving your written request. California patients may inspect records within 5 business days and receive copies within 15 days.
